How do I convert albums with AlbumName (Disc *)

I have many old rips that were done with EAC. EAC was unable to add disc numbers to a disc number field, so the disc number is included as part of the album name:

AlbumName (Disc *)

I want to change these so that they are now

AlbumName

And put the disc number into the Disc meta-tag.

Any easy way to do this?
