What criteria does TuneFUSION use when determining whether a file needs to be synced or not?

Last modified, or you can enable the filesize check.

So, any tag editing with preserve date/time enabled and, with padding, TuneFUSION won't sync these files?

No, there is no way without reading each file fully to know there have been changes.
