What file format? What app are you using on the iPhone?

Please confirm that the artwork is embedded, using Edit ID-Tag?

Perhaps there is an issue with the art, it might be too big (x and y) or too large size wise, or a type of jpeg the iPhone cannot handle such as progressive encoded.

You can set music converter with the DSP effect 'ID Tag Processing' to resize if over a certain pixel or KB size.

If tags and artwork are all ok in iTunes, then the issue is likely to be with the app on the iPhone.
