Error message: "Unable to access 'file name'

This issue has come up far too often and I can't find a work around. When trying to convert files from either an MP3 at a certain bitrate to an MP3 at 192 or a certain file type to an MP3 at 192 I've often received the following error: "Unable to access 'file.mp3', possibly media player has this file open, read support FAQ." I've tied closing out all programs that may possibly have their hooks into audio files and have restarted with minimal programs starting but continually receive the error.

    Any ideas?

Did you check the option to delete the file after conversion? Are you sending the output to the original folder?
