How does dBpoweramp assign a folder to an album?

I've ripped a couple of hundred CDs, most of them automagically got a folder for each artist and sub-folders for each album (eg, MusicFolder>The Beatles> Help). More recently, each song was put in the main Music Folder which has created quite a mess with the album art (the Folder.jpg gets changed with each album ripped and last album ripped is the art that is displayed). I'm ripping into FLAC and have tried, for Output Location, both Folder and Dynamic. Either way gives me the same undesired result. What setting have I inadvertently altered?

Use CD Ripper, then for Naming click Set, then click Default.

If you have the dynamic option it suggests that CD Ripper is not being used.
