Need Help CD Ripping-Dual Messages

I have gotten twice now on diff CD's the Information screen saying all tracks ripped with accuracy and the Error screen saying failed to create output file, which is what actually happened. Could some one explain this ? Thks.

It is possible for the tracks to verify accurate rip (which is reading the cd) yet fail to write the output file

What would cause a failure of the output file, i've ripped a bunch of CDs w/no problems.
