Software is not over writing all files

I am seeing a strange problem. I ripped 2000 CDs in 2011. I recently started ripping some of them again. The issue is when you rip again it wants to overwrite the old files. The problem is it only over writes some of them and some are duplicated. It seems to be random on this and how many. I then have to go manually into all the folders to delete duplicates which were not over written. Why is this happening? I am using 14.4 as my OS software is 32 bit.

    PS
I am writing a wav file to a mapped drive on my music server if that makes any difference.

It is likely the metadata is slightly different and the duplicates are because there are new files being written.
