    The results look consistent to me. In the 2nd example, it looks like the Artist tag was empty/blank before being ripped.

    The structure is defined by your Path and Naming settings.

    In MP3Tag, I tend to use the Extended Tags... dialog.

    First backup your tags using the Audio Info Utility Codec (or Export in MP3Tag).

    Then use the ID Tag Update Utility Codec > Deletions tab.

    You could also check whether a firmware update is available for your drive.

    Discogs and the Pre-Emphasis user database are reporting CP35-3016 as being mastered with pre-emphasis.

    You could rip the CD without de-emphasis and use foobar2000 with the de-emphasis component...
