The following is a list of information that we need to help find the source of your problem and either fix it or note it as a bug.

Windows Version
If you are running as an Administrator
The version of dMC/dAP you are using. Version numbers can be found at Start --> Programs --> dBpoweramp Music Converter --> Configuration --> dBpoweramp Configuration (called dMC Configuration in pre-r11.5 versions)
Format you are converting to, along with bitrates, frequency, channels, etc.
Format you were converting from, along with bitrates, frequency, channels, etc.

If you are ripping a CD, are using the CD ripper/"Audio CD Input" or dMC itself.
If you are ripping a CD, does the error happens on multiple CDs/files or just a few particular ones
For CD ripping, if you have AccurateRip enabled and whether the drive is internal or external.
For CD ripping, if your drive is IDE/Parallel ATA/UltraATA or SATA.

If you are having trouble with dMC, you should consider updating to the latest version (currently dMC r12.1) and see if the problem still exists.

We also need the exact error message, if one is given, and what you were doing when it crashed.

The more details you give us, the faster we can determine the problem.
